If you are a Nurse Practitioner passionate about home health, this per diem opportunity offers flexible scheduling and the ability to impact patient outcomes right in the community. You will conduct focused, 45-minute home health risk assessments for Medicare Advantage members residing across various areas in St. Joseph County. Individual shifts may take you to South Bend, Mishawaka, Osceola, and other nearby locations.

Flexible shifts available any day, Monday through Sunday Self-schedulingdesign shifts around your availability Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

Complete annual home health risk assessments for members of a Medicare Advantage health plan Review and evaluate patients prior/current medical and medication history Educate patients on medical conditions and prescribed treatments Address gaps in care, making recommendations as needed No treatment plan development or prescribing responsibilities

Qualifications:

Active Nurse Practitioner license in Indiana Current BLS/CPR certification Board Certification through AANP or ANCC At least 6 months of recent NP experience preferred MSN degree from an accredited program This role provides competitive per-evaluation compensation, ranging from $80 to $90 for each assessment you complete, in addition to mileage reimbursement and pay for administrative time.
