Key Responsibilities:

Conduct thorough annual or post-discharge home health risk assessments Review prior and current medical history, medical conditions, and medication adherence Educate members on their healthcare conditions and prescribed medications Identify gaps in care and provide actionable recommendations Focus exclusively on assessmentno treatment planning or prescribing required

Qualifications:

Active Nurse Practitioner license in Indiana Current CPR certification (BLS) Board Certification through AANP or ANCC At least 6 months of experience as a practicing NP preferred MSN from an accredited nursing program This position provides compensation of $80-$90 per completed evaluation, in addition to mileage reimbursement and paid administrative time.
