Key Responsibilities:

Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members during scheduled home visits Carefully review patients prior and current medical histories, including conditions and medication usage Educate members on their medical conditions and the importance of proper medication management Identify potential care gaps, communicating recommendations to improve health outcomes Focus solely on assessmentsno prescribing or treatment planning required

Qualifications:

Active Nurse Practitioner license in Indiana Current CPR certification (BLS) Board Certification through
