Role Description This is a part-time/PRN on-site role for a Visit Nurse at Elite Care Management. As a Home Health Visit Nurse, you will be responsible for providing compassionate and personalized care to patients in their homes. Your day-to-day tasks will include conducting assessments, administering medications, coordinating with other healthcare professionals, and educating patients and their families on care plans.

Referral Bonus Program Responsibilities:

Provide comprehensive patient care during shifts, including catheterization, administering medication, and dressing changes. Conduct thorough patient assessments and monitor vital signs to ensure safety and well-being. Utilize EMR systems for accurate documentation and maintain compliance with health regulations. Collaborate with healthcare teams to develop and implement individualized care plans. Administer basic life support and respond to emergencies as needed. Ensure a clean and safe environment for patients. Ability to sit, stand, and kneel as needed to perform physical assessment Ability to work independently and part of a team Requirements Indiana Registered Nurse (RN) license Experience preferred in home health care or a similar setting & Oasis Documentation Strong communication and interpersonal skills Knowledge of medical equipment and procedures Compassionate and empathetic demeanor Attention to detail and organizational skills Valid driver's license CPR Certification Benefits 401(k) Dental insurance Health insurance Paid time off Referral program Travel reimbursement
