Are you an experienced RN who loves patient care but is also ready to use your skills in leadership, education, care coordination, and clinical quality? BrightStar Care of Lake County is seeking an experienced Registered Nurse for a unique hybrid position combining approximately 50% field nursing and 50% office-based clinical leadership and support. This isn't a visit-only home care position. You'll continue building meaningful relationships with clients while also helping develop caregivers, coordinate new referrals, work with pharmacies and healthcare partners, support clinical quality, and help ensure our entire team delivers the level of care our clients deserve. What You'll Do Provide Exceptional Clinical Care You'll maintain an active clinical presence with our clients while developing meaningful, ongoing relationships.
Responsibilities include:
Perform nursing assessments, admissions, reassessments, and supervisory visits Provide skilled nursing care in clients' homes Manage and support infusion therapy patients Perform wound care and monitor healing progress Develop, implement, and update individualized plans of care Recognize changes in client condition and coordinate appropriate interventions Communicate with physicians, pharmacies, clients, families, and other members of the healthcare team Provide clear client and family education regarding medications, treatments, and ongoing care needs Maintain clinical oversight of assigned clients and help ensure care is being delivered safely and appropriately Provide clinical support and backup to other nurses when assistance is needed This is a full-time, Monday-Friday position . Flexibility is required, as client assessments, admissions, supervisory visits, and other clinical visits may occasionally need to be completed during evenings or weekends to accommodate client needs and support timely start of care. You'll:
Conduct caregiver competency evaluations and skills assessments Assist with the clinical portion of caregiver orientation Educate HHAs and caregivers on client-specific care needs Provide ongoing clinical coaching and support Identify skill gaps and provide education or remediation Reinforce care plans, safety expectations, and clinical standards Serve as a trusted clinical resource for caregivers in the field Help create a culture where quality, accountability, and exceptional care are everyone's responsibility Build Strong Clinical Relationships Excellent home care requires more than strong clinical skills—it requires strong relationships.
You'll:
Build trusted relationships with clients and families Develop positive, supportive relationships with caregivers Communicate effectively with pharmacies regarding infusion patients, medications, supplies, orders, and clinical concerns Maintain productive relationships with physicians and other healthcare professionals Collaborate with referral partners and outside clinical teams Follow concerns through to resolution and help close the loop on communication Help create an exceptional experience for clients, caregivers, pharmacies, and healthcare partners Support New Referrals & Clinical Operations Approximately half of this position is office-based, providing the opportunity to develop experience beyond traditional bedside or field nursing.
You'll:
Help facilitate new referrals from clinical review through start of care Assist in determining the clinical and caregiver needs of new clients Help ensure new cases are clinically ready to be staffed Review clinical documentation for completeness, accuracy, and quality Process and track physician orders Maintain accurate and compliant client clinical records Assist with care plan updates Support quality assurance and clinical compliance Collaborate with scheduling, recruiting, and leadership to promote continuity of care Serve as a clinical resource for the office team Work closely with the Director of Nursing on clinical priorities, client needs, and quality initiatives Who We're Looking For Experience matters, but how you approach your work matters just as much. We're interested in experienced nurses who demonstrate strong clinical judgment, leadership potential, excellent communication, adaptability, and a genuine commitment to quality care. Qualifications Required Active, unrestricted Indiana Registered Nurse (RN) license Current CPR certification Valid driver's license Reliable transportation and current auto insurance Strong communication, organizational, and time-management skills Ability to work independently in clients' homes Ability to lift up to 50 pounds and assist with client mobility Preferred Home care, infusion, hospice, case management, or community nursing experience Infusion therapy experience Wound care experience Clinical leadership or supervisory experience Experience educating clients and families Experience training, mentoring, or evaluating HHAs, caregivers, or clinical staff Experience coordinating care with pharmacies, physicians, or other healthcare providers Schedule & Compensation $40
