Key Responsibilities:

Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members in their homes Evaluate patients prior and current medical histories, review medical conditions and medication histories Educate patients on health conditions and proper medication management Identify care gaps, providing clear, actionable recommendations to facilitate quality patient care No requirement for developing treatment plans or prescribing medications

Qualifications:

Current Nurse Practitioner license in Indiana BLS (CPR) certification Board certification through AANP or ANCC At least 6 months of nurse practitioner experience preferred Master of Science in Nursing (MSN) from an accredited school Enjoy competitive compensation of $80$90 per evaluation, with a mileage stipend and paid administrative time included.
