Key Responsibilities:

Complete comprehensive annual home health risk assessments in member homes Review and evaluate prior/current medical history, ongoing conditions, and medication use Offer education regarding health conditions and prescribed medications Detect and document gaps in care, providing actionable recommendations for improvement No obligation for developing treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in Indiana Current certification in Basic Life Support (CPR/BLS) Board certification via AANP or ANCC Minimum six months of recent Nurse Practitioner experience preferred MSN degree from an accredited nursing program This role delivers competitive compensation of $80$90 for each evaluation completed, plus a mileage stipend and additional pay for administrative time.
