Seeking a skilled Nurse Practitioner with experience in home health to perform risk assessments for Medicare Advantage members across St. Joseph County, IN. This per diem opportunity offers flexibility and autonomy, allowing you to create your own schedule as you conduct visits in South Bend, Mishawaka, Osceola, and surrounding areas. Each assessment takes approximately 45 minutes and may include annual wellness visits or post-discharge check-ins. Schedule/Hours Flexible shifts available between Monday and Sunday Self-scheduling to suit your availability Minimum weekly or bi-weekly hourly commitment required Key Responsibilities Perform in-home risk assessments for Medicare Advantage plan members Review and evaluate each patients medical, surgical, and medication history Assess current health conditions and address patient concerns Educate patients on disease management, prevention, and medication adherence Identify gaps in care and document recommendations for improvement No treatment planning or prescription authority required Qualifications Active Nurse Practitioner license in Indiana Current CPR certification (BLS) Board certification through AANP or ANCC At least 6 months of recent experience as a Nurse Practitioner, preferably in home health MSN from an accredited nursing school In this role, youll earn $80-$90 per evaluation, plus receive a mileage stipend and compensation for administrative time.
