The RN will work closely with the Administrator and care team to ensure clients receive appropriate, safe, person-centered services and that clinical needs are properly assessed, documented, and communicated. This position is ideal for an RN who enjoys combining clinical knowledge with care coordination and administrative responsibilities.
Key Responsibilities:
Conduct initial and ongoing client assessments to identify health, safety, functional, and care needs. Assist with the development, review, and updating of individualized client service plans based on assessment findings and client needs. Assist the Administrator with ensuring services are appropriate to the client's needs and within the scope of services provided by the agency. Monitor clients for changes in condition, needs, or circumstances that may require changes to the service plan. Assist with coordinating appropriate changes to services and communicating those changes with clients, families, caregivers, and members of the care team. Promote continuity of care by ensuring important client information is communicated appropriately among staff and other involved healthcare professionals. Provide clinical guidance and support to direct-care staff when appropriate and within the RN's scope of practice. Participate in the orientation, education, and training of employees as needed. Review documentation to help ensure services are being provided according to the client's service plan and agency policies. Assist with monitoring client health and safety and identifying concerns that require follow-up. Communicate with physicians, healthcare providers, family members, and other professionals when appropriate and authorized. Assist with coordinating care following hospitalizations, emergency room visits, rehabilitation stays, or other significant changes in a client's condition. Assist the Administrator with quality assurance, clinical documentation, and regulatory compliance activities. Serve as the Alternate Administrator and provide administrative support and coverage when the Administrator is unavailable, as assigned. Participate in agency meetings, care conferences, quality improvement activities, and other administrative functions as needed. Maintain accurate, timely, and confidential client records in accordance with agency policies and applicable laws and regulations. Protect client privacy and confidentiality at all times. Perform other duties consistent with the RN's qualifications, agency needs, and applicable scope of practice.
Qualifications:
Current, unrestricted Indiana Registered Nurse (RN) license . Previous experience in home health, homecare, case management, care coordination, geriatrics, community health, or a related setting . Strong assessment and clinical documentation skills. Knowledge of person-centered care and individualized service planning. Ability to recognize changes in a client's condition and communicate concerns appropriately. Excellent communication, organization, and interpersonal skills. Ability to work independently while also collaborating effectively with the agency's administrative and care team. Reliable transportation and the ability to travel to client homes when required. Ability to maintain professional boundaries and protect confidential client information. Previous supervisory or administrative experience is a MUST.
Pay:
$40.00 - $43.00 per hour
