Responsibilities:

Conduct home health risk assessments for Medicare Advantage plan members in Cass County and surrounding areas, with each assessment averaging 45 minutes Evaluate prior and current medical histories, ongoing health conditions, and medication regimens Deliver clear education to patients about their health conditions and medication usage Identify gaps in care, offering practical recommendations for improvement Visits may include annual health assessments or post-discharge evaluations No requirements for treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in Indiana Current CPR certification (BLS) Board Certified by AANP or

ANCC MSN

degree from an accredited nursing program At least six months of recent Nurse Practitioner experience is preferred Enjoy per evaluation pay between $80-$90, along with a mileage stipend and compensation for administrative time associated with each visit.
