Key Responsibilities:

Perform thorough annual home health risk assessments for Medicare Advantage enrollees Review prior and present medical history, conditions, and medication use with each patient Offer clear education on ongoing medical concerns and medications to patients in their homes Detect gaps in care and recommend solutions, supporting better health outcomes No requirement to prescribe medication or develop treatment plans

Qualifications:

Active Nurse Practitioner license in Indiana Board Certification via AANP or

ANCC MSN

from an accredited nursing program Current BLS (CPR) certification At least 6 months of nurse practitioner experience preferred Expect compensation of $80-$90 for each evaluation you conduct, with additional mileage stipend and administrative time covered.
