Job Description

Step into a dynamic per diem opportunity as a Nurse Practitioner specializing in home health risk assessments throughout Fayette County, IN. This position offers unmatched schedule flexibility, allowing you to shape shifts around your availability and tailor your weekly or bi-weekly hours to fit your lifestyle. With a focus on Medicare Advantage members in various local communities, your expertise will make a meaningful difference across Connersville, Glenwood, Bentonville, and surrounding areas.
Schedule:
Flexible shifts available Monday through Sunday Self-schedulingcreate your own hours Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members during visits averaging 45 minutes Evaluate and review patients prior and current medical history, ongoing conditions, and medication profiles Deliver clear education to patients regarding their medical conditions and prescribed medications Identify care gaps and offer recommendations to enhance care, without the need to create treatment plans or prescribe medications
Qualifications:
Active Nurse Practitioner License in Indiana Current CPR certification (BLS) Board Certification through AANP or
ANCC MSN
from an accredited nursing program Six months or more of experience as a Nurse Practitioner is preferred Enjoy competitive compensation with $80$90 per completed evaluation, including mileage stipends and compensation for administrative time.
