Key Responsibilities:

Conduct comprehensive annual health risk assessments in members homes within Knox County, IN Assess prior/current medical history, chronic conditions, and medication profiles Educate patients regarding their medical conditions, treatments, and medication management Identify gaps in care and make recommendations for improvement No prescribing or development of ongoing treatment plans required

Qualifications:

Active Nurse Practitioner license in the state of Indiana Board Certification through AANP or

ANCC MSN

degree from an accredited nursing program Current Basic Life Support (BLS/CPR) certification Minimum of six months' experience as a Nurse Practitioner (preferred) Enjoy competitive per evaluation earningsranging from $80 to $90 for each assessment completed, along with a stipend for mileage and compensation for administrative time directly related to the role.
