Responsibilities:

Conduct annual and post-discharge home health risk assessments for Medicare Advantage members within Grant County, IN. Evaluate prior and current medical history, medical conditions, and medication usage. Educate patients on their conditions and medications in a home environment. Identify gaps in care, providing recommendations to close them. Focus on assessment; no treatment plan creation or prescribing medications required. Each home assessment lasts about 45 minutes and may occur in Marion, Gas City, Upland, and other Grant County locales.

Qualifications:

Active Nurse Practitioner license in Indiana (IN) Current CPR certification (BLS) Board Certified through AANP or ANCC Master of Science in Nursing (MSN) from an accredited program Minimum six months of recent NP experience preferred
