Schedule:

13-week contract with potential for extension Full-time, Monday through Friday On-call responsibilities may be included Structured daily visit requirements

Key Responsibilities:

Deliver skilled in-home nursing care, including wound care, medication management, and disease monitoring Conduct comprehensive patient assessments and complete OASIS documentation accurately and timely Implement and revise individualized care plans in coordination with physicians and care teams Provide patient and caregiver education to support optimal outcomes Maintain clear communication regarding patient status and changes to the clinical team

Qualifications:

Active RN license Minimum of 1 year recent home health RN experience OASIS proficiency required Experience with HomeCare HomeBase strongly preferred Reliable transportation and a valid drivers license Demonstrated ability to follow a structured, full-time visit schedule
