Key Responsibilities:

Perform annual home health risk assessments for members of a Medicare Advantage health plan Review and evaluate patients previous and current medical history, ongoing conditions, and medication usage Offer education to patients regarding chronic conditions, general health, and medication regimens Detect gaps in care and recommend solutions for improved health outcomes Visits do not include the creation of treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in IN Board Certification via AANP or ANCC Current CPR certification (BLS required) MSN degree from an accredited nursing program At least 6 months of current Nurse Practitioner experience preferred You will be serving patients in areas throughout Vigo County, including Terre Haute, West Terre Haute, and Seelyville. Each home health visit typically lasts about 45 minutes. Compensation for this position ranges from $80 to $90 per completed evaluation, plus a mileage stipend and additional payment for administrative time.
