Role:
As an MDS Coordinator, you will be responsible for: Conducting MDS assessments, developing and implementing care plans, and coordinating care with interdisciplinary teams Assuring accurate and thorough documentation of patient assessment data, care plans, and care coordination activities Identifying and addressing potential barriers to care, and implementing strategies to improve patient outcomes Collaborating with healthcare professionals, caregivers, and families to ensure comprehensive and individualized care Maintaining accurate and up-to-date records, reports, and statistical data Participating in quality improvement initiatives to enhance patient care and outcomes
Responsibilities:
Coordinate and conduct MDS assessments, care planning, and care coordination activities Ensure accurate and timely completion of MDS assessments and care plans Develop and implement effective care plans that address patient needs and outcomes Collaborate with healthcare professionals, caregivers, and families to ensure comprehensive and individualized care Identify and address potential barriers to care, and implement strategies to improve patient outcomes Maintain accurate and up-to-date records, reports, and statistical data
Requirements:
Bachelor's degree in a related field (e.g., nursing, social work, healthcare administration) 1+ years of experience in MDS coordination, healthcare administration, or a related field Strong analytical, communication, and organizational skills Ability to work effectively in a fast-paced environment with multiple priorities Strong attention to detail and accuracy in data collection and documentation
