Coordinates the interdisciplinary team in resident assessment and development of resident plan of care according to the Resident Assessment Instrument (RAI) and state and federal requirements.Job Responsibilities:
Assess residents in order to accurately complete the Minimum Data Set and Resident Assessment Protocol.Coordinate MDS and Care Plan Team Implement and monitor individual care plans for residents based on MDS Guidelines.
