Our RN Case Managers are essential in aligning patient needs with appropriate medical services through comprehensive assessment and planning, utilizing evidence-based practices. Responsibilities Assess patient needs and develop individualized care plans in collaboration with a multidisciplinary team. Facilitate timely coordination of quality healthcare services to meet an individual's health needs across the continuum of care. Advocate for patients and families to ensure that care is patient-centered and aligns with their values and needs. Monitor patient progress and outcomes, adjusting care plans as needed. Communicate effectively with all stakeholders, including patients, families, healthcare teams, and insurance providers. Ensure compliance with healthcare regulations and standards.
