Primary Responsibilities:

Deliver direct in-home nursing care and skilled interventions Conduct comprehensive patient assessments and complete OASIS documentation with accuracy and timeliness Collaborate effectively with physicians and interdisciplinary care teams to develop and implement care plans Manage wound care, medication administration, disease monitoring, and patient/family education Communicate patient condition updates promptly to the clinical team

Qualifications:
