Responsibilities:

Conduct thorough assessments of patients' physical, emotional, and social needs within their home environments. Develop personalized care plans aligned with medical directives and social service resources to address each client's unique circumstances. Administer medications, perform wound care, and monitor vital signs with precision and compassion. Educate clients and their families on health management, medication adherence, and wellness strategies. Maintain detailed documentation of care provided, progress notes, and communication logs in compliance with healthcare regulations such as Medicaid and Medicare standards.

Qualifications:

Valid Registered Nurse (RN) license in good standing with current state requirements. Certification in

CPR and First Aid Additional Information:

Agency vehicle is provided to complete daily visits and when on-call Hours are Monday - Friday 8am - 4pm 11.5 paid Holidays per year Background check and drug screening is required

Pay:

From $30.00 per hour
