Schedule:

13-week contract, with the possibility of extension Full-time, Monday through Friday Likely on-call responsibilities

Key Responsibilities:

Provide in-home nursing care, skilled interventions, and comprehensive assessments Complete OASIS documentation accurately and promptly for all patient interactions Collaborate with physicians, therapists, and other care team members to establish and execute individualized care plans Deliver wound care, medication management, disease monitoring, and patient/family education Maintain thorough communication on patient status and updates with the broader clinical team

Qualifications:

Active Registered Nurse (RN) license At least one year of recent home health nursing experience Strong proficiency in OASIS documentation (required) Familiarity with HomeCare HomeBase software is strongly preferred Reliable transportation and a valid driver's license Able to confidently manage a structured daily visit schedule
