As Stroke Program Coordinator at TriStar Health, you will lead an evidence-based stroke program that supports rapid response, standardized care, and excellent outcomes. You will coordinate stroke workflows from ED arrival through discharge, monitor quality metrics, and ensure compliance with regulatory and accreditation standards. In this role, you'll serve as a clinical resource to physicians and staff, provide stroke-related education, and drive performance improvement initiatives. Partnering with multidisciplinary teams, you will refine protocols, reduce treatment times, and enhance patient safety. You'll also support patient and family education, community outreach, and staff competency development. Responsibilities Coordinate and oversee the stroke program to ensure compliance with evidence-based guidelines and accreditation standards Serve as a clinical stroke resource for physicians, nurses, and interdisciplinary teams across the facility Monitor stroke quality metrics, conduct chart reviews, and lead performance improvement initiatives Educate staff, patients, and families on stroke recognition, treatment pathways, and post-discharge care Collaborate with emergency services, neurology, radiology, and ICU to optimize stroke response times and care transitions Lead stroke drills, debriefs, and process reviews to enhance patient safety and outcomes Develop and update stroke policies, protocols, and order sets according to best practices and regulatory changes Prepare and present stroke program data, outcomes, and action plans to leadership and committees Support community outreach and education on stroke prevention and early recognition Precept and mentor nurses in stroke competencies and promote a culture of continuous learning Required Skills Acute stroke clinical care Neuro assessment and monitoring Quality improvement and data analysis Electronic medical record (EMR) documentation Care coordination and discharge planning Policy and protocol development Staff education and clinical training Regulatory and accreditation compliance (e.g., Joint Commission stroke standards) Performance metric tracking and reporting Interdisciplinary communication and collaboration
