Full-Time Join Our Team ContinueCARE Hospital at Baptist Health is seeking a compassionate, collaborative, and patient-focused Registered Nurse (RN) Case Manager or Licensed Social Worker to join our interdisciplinary team. In this vital role, you will coordinate patient care, discharge planning, and community resource management to ensure exceptional outcomes for our patients and their families. Position Summary The RN Case Manager/Licensed Social Worker directs and coordinates case management services by integrating patient care planning, discharge planning, utilization management, and community resource coordination. This position works closely with physicians, nursing staff, patients, families, and community agencies to facilitate safe, effective, and timely transitions of care. Responsibilities Coordinate comprehensive patient care, discharge planning, and post-acute care services. Complete psychosocial assessments for new admissions within established timeframes. Develop individualized care plans in collaboration with physicians, nurses, therapists, and interdisciplinary team members. Assist patients and families in addressing the social, emotional, and psychological effects of illness. Coordinate referrals to rehabilitation facilities, skilled nursing facilities, home health agencies, hospice, community resources, and other post-discharge services. Educate patients and families regarding discharge plans, recovery expectations, and available resources. Participate in interdisciplinary care conferences and discharge planning meetings. Monitor utilization review activities, length of stay, and medical necessity. Serve as the primary liaison between the hospital and community agencies. Participate in quality improvement initiatives and departmental performance improvement activities. Assist with departmental reporting, program development, and committee participation. Promote patient rights, dignity, confidentiality, and compliance with all regulatory standards. Qualifications Education Bachelor's degree in Nursing, Social Work, or a related healthcare field preferred. Experience Minimum of two (2) years of healthcare, hospital, or LTAC experience preferred. Experience in case management, discharge planning, utilization review, or care coordination is highly desirable. Licensure Current, unrestricted Registered Nurse (RN) license or Licensed Social Worker credential in the state of employment. BLS Certification required within 30 days of hire. Skills Strong clinical assessment and critical thinking abilities. Excellent communication and interpersonal skills. Ability to work effectively within an interdisciplinary healthcare team.
