A Registered Nurse provides medical care and treatment, educates patients about their conditions, and provides emotional support to patients and families. Works in hospitals, schools, clinics or community health centers. Works under the supervision of a physician or other specialist, and serves as the primary point of care for patients in a hospital setting. May specialize in emergency room work, or treatment for a particular condition, or specialize in working with infants, the elderly or other patient groups. May work as an advanced practice specialists and prescribe medications in addition to offering specialty care.

Schedule:
First shift, full-time hours. Approximately 20% of your work will involve home visits. Specific work hours will be established in consultation with your supervisor.

Duties/Responsibilities:
Provide direct and indirect nursing care to participants following the Care Plan. Collect and analyze patient data, set priorities, and identify problems needing follow-up. Develop, implement, and evaluate plans of care, collaborating with interdisciplinary teams. Provide education, guidance, and discharge instructions to participants and families. Respond independently to emergencies and implement advanced care as needed. Document care, observations, and participant outcomes accurately in the electronic medical record. Communicate participant changes and updates clearly to the care team. Participate in Quality Improvement initiatives and clinical process enhancements. Education and Experience:
Must be a graduate of an accredited nursing school and have a current RN license within practicing state. Experience in complex nursing care and interdisciplinary collaboration preferred. Experience working with the frail or elderly population is a plus — but not required. We'll provide hands-on training and mentoring to help you grow!

Must be able to move intermittently throughout the workday. Must function independently, maintain confidentiality, have flexibility, personal integrity, and the ability to work effectively with personnel, regional and corporate consultants, vendors, patients/participants and support agencies.
