Role Overview As our MDS Coordinator, you will lead the comprehensive assessment process with accuracy and care, working closely with our interdisciplinary team to create meaningful, resident-centered care plans. Your work will directly impact the quality of life for those who call Tradewater home, supporting continuous improvement and fostering a culture where respect and accountability are fundamental. Key Responsibilities Lead the MDS/RAI process, ensuring accuracy, timeliness, and compliance with regulations Complete assessments that genuinely reflect each resident's needs, strengths, and goals Collaborate with the care team to develop personalized care plans that honor residents' preferences and dignity Support facility-wide quality improvement initiatives and promote positive change Monitor quality measures and maintain documentation that supports excellent outcomes Participate actively in care plan meetings and communicate clearly with staff and leadership Foster a culture where thorough documentation, accountability, and respect for residents are non-negotiable Qualifications Active Kentucky RN (required) Previous MDS/RAI experience (required) Strong knowledge of PDPM, care planning, and regulatory standards Dedicated to quality, accuracy, and ongoing improvement Excellent communication and teamwork abilities A genuine passion for supporting a resident-first culture in a home-like atmosphere Why Join Us?
