Shift Job Summary:
The Nurse Navigator, under the direction of the Practice Manager, serves as the primary clinical point of contact for specialty care patients and their families, providing comprehensive coordination across the continuum of care. This role leverages deep specialty-specific knowledge to guide patients from diagnosis through treatment, discharge, and follow-up, eliminating barriers to timely care, facilitating system flow, and ensuring seamless transitions between referring providers, specialists, clinical research teams, ancillary services, and clinical outcomes departments. Acting as educator, advocate, and care coordinator, the Nurse Navigator supports informed decision-making, reinforces treatment plans, promotes adherence, and enhances both patient and provider satisfaction by delivering proactive, patient-centered navigation within a complex specialty care environment. Minimum Education, Training, and Experience Required Bachelor's degree in Nursing, or higher. Current Kentucky or Compact State unrestricted license as a Registered Nurse. Must maintain an active BCLS and ACLS card through the American Heart Association.
