Job Description

Encompass Health Cardinal Hill Rehabilitation Hospital seeks a compassionate Registered Nurse to join our Healthcare & Medical Services team. In this inpatient rehabilitation setting, you'll deliver evidence-based, patient-centered care, coordinate interdisciplinary treatment plans, and support patients through recovery and functional improvement. You'll collaborate closely with therapists and physicians, prioritize safety and quality outcomes, and mentor support staff. Responsibilities Provide direct, evidence-based nursing care to adult rehabilitation patients, including assessments, interventions, and evaluations Develop, implement, and regularly update individualized nursing care plans based on patient conditions and rehabilitation goals Administer medications, treatments, and therapies safely and accurately, following hospital policies and provider orders Monitor and document patient status, vital signs, functional progress, and responses to interventions using electronic health records Collaborate with physicians, therapists, case managers, and other team members to coordinate comprehensive, interdisciplinary care Educate patients and families about diagnoses, treatment plans, rehabilitation expectations, safety, and discharge instructions Promote patient safety by following infection control standards, fall prevention strategies, and hospital safety protocols Participate in care conferences, shift handoffs, and team meetings to communicate changes in patient condition and care priorities Contribute to quality improvement and evidence-based practice initiatives aimed at enhancing patient outcomes and clinical processes Support and mentor peers and support staff, fostering a collaborative, respectful, and patient-focused unit culture Required Skills Patient assessment and clinical judgment Medication administration and IV therapy Rehabilitation nursing care and discharge planning Wound care and skin integrity management Vital signs monitoring and hemodynamic awareness Care planning and care coordination Electronic health records (EHR) documentation Interdisciplinary team collaboration Patient and family education Infection prevention and control Time management and prioritization Safe patient handling and mobility support Basic cardiac and respiratory monitoring Critical thinking and problem-solving Professional communication and de-escalation techniques
