Summary Job Description:
Baptist Health Medical Group Nurse Navigator Memory Disorders Neurology LaGrange, Ky Full Time/First Shift The Nurse Navigator serves as the primary point of contact for physicians and other healthcare providers, patients and their caregiver/family, and other resource-based entities, providing and accessing clinical and supportive care services offered within the multidisciplinary teams at Baptist Health Hardin and other entities as needed. The Nurse Navigator serves as an integral part of the care team within the Baptist Health Medical Group coordinating effective and comprehensive services through the continuum of care for patients with treatment and support service options. The Nurse Navigator serves as an educator, advocate, and communicator for the patient and patient caregiver/family and proactively develops processes that streamline and improve the clinical experience for both the patient and their healthcare provider(s). The Nurse Navigator will demonstrate through organizational skills the ability to provide ease and patient safety in the operational flow of the accessed Baptist Health Hardin departments. Nurse Navigator demonstrates the broad-based knowledge, skill, and coordination to provide nursing care and guidance to our pulmonary patients. This position requires a collegial and collaborative working style to fulfill these specific work functions.
Minimum Education, Experience, Training and Licensures Required :
Graduate of an accredited School of Nursing with current RN licensure by the Commonwealth of Kentucky State Board of Nursing required. Bachelor of Science in Nursing required, or higher.
