Schedule & Assignment Details:

13-week contract with the possibility of extension Monday through Friday full-time hours (on-call responsibilities likely) Structured daily visit schedule

Key Responsibilities:

Perform in-home nursing care and skilled procedures for patients Complete comprehensive patient assessments and precise OASIS documentation Collaborate effectively with physicians and interdisciplinary care teams Deliver wound care, manage medications, educate patients and families, and monitor disease progression Clearly communicate any patient changes or updates to the clinical team

What Were Looking For:

Current and active RN license At least 1 year of recent home health RN experience Demonstrated OASIS documentation skills (required) Strong familiarity with HomeCare HomeBase (preferred) Ability to reliably manage a full-time, structured patient visit schedule Valid drivers license and dependable transportation
