Schedule:

13-week contract with the possibility of extension Full-time, Monday through Friday On-call coverage is probable What youll do: Deliver skilled nursing care to patients in their homes, ensuring professional standards are met Complete comprehensive patient assessments and OASIS documentation with accuracy and timeliness Support care coordination by collaborating with physicians and interdisciplinary teams to implement individualized care plans Perform tasks including wound care, medication management, patient/family education, and ongoing disease monitoring Communicate patient status, changes, and updates clearly to the broader clinical team

Qualifications:
