Key Responsibilities:

Deliver in-home nursing care and skilled interventions Complete thorough patient assessments and OASIS documentation accurately and promptly Collaborate with physicians and interdisciplinary care teams to develop and implement individualized patient care plans Administer wound care, manage medications, provide disease monitoring, and foster patient education Consistently communicate patient status updates to the clinical team

Desired Qualifications:

Active RN license (required) At least 1 year of recent RN experience in home health settings Demonstrated proficiency with OASIS documentation Familiarity with HomeCare HomeBase strongly preferred Dependable transportation and a valid drivers license Organized, self-motivated, and comfortable managing a full-time clinical visit schedule Enjoy highly competitive weekly pay, direct deposit, and access to robust health benefits.
