Responsibilities:

Conduct comprehensive in-home assessments of patients physical, psychosocial, and environmental needs Develop and continually update individualized care plans to reflect patient status and physician directives Implement physician-ordered interventions, monitor patient progress, and provide skilled nursing care including medication administration, IV therapy, and wound management Educate patients and families on disease management, treatment procedures, and safe practices in the home setting Documentation of all assessments, interventions, and visits in compliance with regulatory standards Communicate and collaborate with interdisciplinary team members to ensure seamless care transitions and optimal patient outcomes This position offers the chance to build meaningful relationships with patients while providing high-quality care in the comfort of their homes.
