Job Description RN Case Manager Mayfield, KY Pay:

13 Weeks RN Case Manager Requirements Active RN license valid in Kentucky Current BLS certification Minimum of one year of nursing experience Recent RN case-management and acute-care experience Strong communication, organization, and clinical documentation skills Ability to collaborate effectively with interdisciplinary care teams Key Responsibilities Coordinate patient care throughout hospitalization and discharge planning. Review clinical information and assess patient needs. Collaborate with physicians, nurses, therapists, social services, payers, and outside providers. Help determine the appropriate level of care and facilitate safe, timely discharges. Identify and resolve barriers that may delay discharge. Arrange post-acute services, equipment, referrals, and community resources. Communicate care plans and available resources to patients and families. Support utilization-review and care-coordination activities. Maintain accurate and timely case-management documentation.
