Schedule:

Key Responsibilities:

Deliver skilled nursing care to patients in their homes Conduct thorough patient assessments and accurately complete OASIS documentation Collaborate closely with physicians and interdisciplinary care teams to develop individualized care plans Provide wound care, manage medications, educate patients, and monitor chronic conditions Promptly communicate changes in patient condition to the clinical team

Qualifications:

Current, active RN license At least 1 year of recent experience as a home health

RN OASIS

documentation experience required Proficiency using HomeCare HomeBase (strongly preferred) Reliable transportation and valid drivers license Able to maintain a structured, full-time visit schedule
