Job Description
Encompass Health Cardinal Hill Rehabilitation Hospital seeks a compassionate Registered Nurse to join our Healthcare & Medical Services team. In this inpatient rehabilitation setting, you'll deliver evidence-based, patient-centered care, coordinate interdisciplinary treatment plans, and support patients through recovery and functional improvement. You'll collaborate closely with therapists and physicians, prioritize safety and quality outcomes, and mentor support staff. Responsibilities Provide comprehensive nursing care to rehabilitation patients, including assessment, planning, implementation, and evaluation of individualized care plans Administer medications, treatments, and therapies safely and accurately, including IV management and wound care Monitor, document, and report patient status, vital signs, responses to treatment, and any significant changes using the electronic medical record Collaborate with physicians, therapists, case managers, and other interdisciplinary team members to coordinate and optimize patient care and discharge planning Educate patients and families on diagnoses, medications, therapies, safety, and home care strategies to support recovery and prevent readmissions Assist patients with mobility, transfers, and activities of daily living, promoting independence and adherence to therapy goals Participate in interdisciplinary rounds, care conferences, and quality improvement initiatives aimed at enhancing patient outcomes and patient safety Adhere to all regulatory standards, infection prevention protocols, and organizational policies while maintaining a safe, clean, and therapeutic environment Support onboarding and mentoring of new staff, nursing students, and support personnel as needed Engage in ongoing professional development, maintaining required licensure and certifications and staying current with evidence-based rehabilitation nursing practices Required Skills Patient assessment and clinical judgment Medication administration and IV therapy Wound care and skin integrity management Rehabilitation nursing principles and functional assessments Care planning and care coordination Electronic medical records (EMR) documentation Vital signs monitoring and clinical observation Patient and family education Interdisciplinary team collaboration Fall prevention and patient safety practices Time management and prioritization in a fast-paced setting Infection prevention and control Pain assessment and management Stroke, orthopedic, neurologic, and cardiac rehab care basics Discharge planning support
