Job Description
Signature HealthCARE is seeking a compassionate Registered Nurse to join our Healthcare & Medical Services team. In this role, you will deliver person-centered nursing care, coordinate with an interdisciplinary team, and support residents in long-term care and post-acute rehabilitation settings. Responsibilities include assessments, care planning, medication administration, documentation, and family communication. Responsibilities Provide direct nursing care to residents and patients, including assessments, treatments, and medication administration. Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team. Monitor changes in resident condition, respond to emergencies, and use sound clinical judgment to escalate concerns. Document all assessments, interventions, and outcomes accurately and timely in the electronic health record (EHR). Educate residents and families on diagnoses, medications, treatment plans, and health maintenance strategies. Ensure compliance with facility policies, state and federal regulations, and infection control and safety standards. Coordinate with physicians, therapists, nursing assistants, and other team members to optimize resident outcomes. Support quality improvement and performance initiatives aimed at enhancing resident-centered care and clinical excellence. Mentor and support nursing assistants and peers by modeling best practices and a culture of respect and teamwork.
