Schedule:

Key Responsibilities:

Deliver skilled in-home nursing care and interventions according to individualized care plans Accurately complete OASIS assessments and documentation in a timely manner Collaborate effectively with physicians and interdisciplinary care teams Perform wound care, medication administration, patient education, and chronic disease monitoring Clearly communicate updates or changes in patient condition to the clinical team

Qualifications:

Active, unrestricted RN license Minimum of 1 year recent experience as a home health RN Expertise in OASIS documentation is required Strongly prefer proficiency with HomeCare HomeBase software Ability to manage daily patient visits and a structured full-time schedule Reliable transportation and a valid drivers license are required
