PM Key Responsibilities:

Perform comprehensive patient assessments using OASIS and develop individualized care plans. Deliver skilled nursing services, including wound care, IV therapy, medication administration, and chronic disease management during home visits. Monitor patient progress, recognize changes in condition, and promptly communicate findings to the broader healthcare team. Educate patients and families on health conditions, medication regimens, and strategies for disease prevention and wellness. Accurately document all care using Home Care Home Base (HCHB). Coordinate care by collaborating with therapists, social workers, physicians, and aides. Attend case conferences, team meetings, and participate in ongoing education. Consistently adhere to agency and regulatory guidelines for patient care and documentation.

Qualifications:

Current, active Registered Nurse license in good standing. At least 1 year of recent home health experience (field-based strongly preferred). Demonstrated proficiency in Home Care Home Base (HCHB) and experience with OASIS assessments. Reliable vehicle, valid drivers license, and current auto insurance. Strong assessment, problem-solving, and organizational skills. Ability to work independently and effectively manage a flexible field-based caseload. BSN preferred. Experience managing a caseload of 2532 points per week is an advantage. Knowledge of Medicare and home health compliance is desirable.
