Essential Qualifications:

Active RN license At least one year of recent RN experience in home health Proven OASIS proficiency Experience with HomeCare HomeBase (strongly preferred) Reliable transportation and valid drivers license Commitment to maintaining a set visit schedule and accurate documentation

Primary Duties:

Deliver skilled nursing interventions and in-home care Complete thorough patient assessments and submit timely, accurate OASIS documentation Collaborate with physicians and interdisciplinary teams to develop and implement care plans Manage wound care, medication administration, patient/family education, and monitor chronic conditions Communicate changes in patient status and coordinate updates with care teams
