Northshore and Southshore Overview:

We are seeking a compassionate and dedicated Hospice Nurse to join our team. As a Hospice Nurse, you will provide specialized care and support to patients and their families during the end-of-life journey. This is a rewarding opportunity to make a meaningful difference in the lives of those facing terminal illnesses.

Duties:

• Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Administer medications and treatments as prescribed by the physician
• Monitor patients' condition and provide symptom management
• Educate patients and their families about the disease process, treatment options, and end-of-life care
• Coordinate with other healthcare professionals to ensure continuity of care
• Maintain accurate and up-to-date medical records and documentation
• Provide emotional support and counseling to patients and their families
• Follow HIPAA guidelines to ensure patient privacy and confidentiality

Skills:

• Proficient in medical terminology, anatomy, and physiology knowledge
• Strong nursing skills with a focus on patient care
• Excellent communication and interpersonal skills
• Ability to provide compassionate end-of-life care
• Attention to detail for accurate medical documentation
