Experienced registered nurses with a background in home health are needed for a 13-week contract opportunity providing skilled nursing care in a field-based role. This position focuses on delivering patient-centered services in the comfort of patients homes, fostering quality, continuity, and compassionate care throughout the course of treatment.

Schedule:

Monday Friday, 8:00 AM 5:00

PM Key Qualifications:

Current RN license in good standing At least 1 year of recent home health RN experience Proficiency with Home Care Home Base (HCHB) OASIS experience required Strong clinical assessment and independent decision-making abilities Reliable transportation, valid drivers license, and active auto insurance Excellent time management, organization, and communication skills BSN preferred Familiarity with Medicare and federal home health regulations a plus Previous experience managing a 2532 point caseload/week encouraged

Responsibilities:

Complete OASIS assessments and develop individualized patient care plans Deliver a spectrum of skilled nursing interventions, including wound care, IV therapy, medication administration, and chronic disease management Monitor progress, communicate changes in patient conditions, and collaborate with physicians and interdisciplinary team members Educate patients and family members on disease management, treatments, medications, and prevention strategies Maintain accurate, timely electronic documentation in HCHB Coordinate with physical therapists, occupational therapists, social workers, and home health aides Attend case conferences and mandatory training
