Schedule & Structure:

13-week contract, with possibility for extension Full-time, Monday through Friday Daily structured visits On-call may be required

Key Responsibilities:

Deliver skilled, compassionate nursing care to patients in their homes Complete comprehensive patient assessments and submit OASIS documentation accurately and on time Develop and implement individualized care plans in collaboration with physicians and the greater care team Perform wound care, patient education, medication management, and disease process monitoring Communicate condition changes and updates to the clinical team promptly

Qualifications:

Active RN license At least 1 year of recent home health RN experience required OASIS experience required Proficiency in HomeCare HomeBase is strongly preferred Demonstrated ability to maintain a structured, full-time schedule with scheduled daily visits Reliable transportation and a valid drivers license
