Full-Time Hospice Specialists of Louisiana is seeking a compassionate, dependable, and experienced Registered Nurse (RN) Case Manager to join our hospice team serving patients throughout the Metairie, Kenner, and New Orleans areas . We are looking for an RN who understands that exceptional hospice care is about more than clinical skills. It is about providing comfort, dignity, education, and support to patients and their families during one of the most important times in their lives. Position Summary The RN Case Manager is responsible for coordinating and providing skilled nursing care to an assigned caseload of hospice patients. The RN works closely with physicians, nurse practitioners, hospice aides, social workers, chaplains, and other members of the interdisciplinary team to ensure each patient receives individualized, high-quality end-of-life care. Responsibilities Manage an assigned caseload of hospice patients in the Metairie, Kenner, and New Orleans areas Perform comprehensive nursing assessments and ongoing evaluations of patient needs Develop, implement, and update individualized plans of care in collaboration with the interdisciplinary team Provide skilled nursing care focused on symptom management, comfort, and quality of life Monitor changes in patient condition and communicate promptly with physicians and other members of the care team Educate patients, families, and caregivers regarding medications, disease progression, symptom management, and what to expect at end of life Coordinate medications, medical equipment, supplies, and other services necessary to support the patient's plan of care Complete accurate and timely clinical documentation Participate in interdisciplinary group (IDG) meetings and contribute to ongoing care planning Coordinate care with facilities, hospitals, physicians, pharmacies, and other healthcare providers as needed Provide emotional support and guidance to patients and families Participate in the agency's on-call rotation as assigned Ensure care is provided in accordance with the hospice plan of care, agency policies, and applicable state and federal requirements Qualifications Current, unrestricted Louisiana Registered Nurse (RN) license Minimum of one year of nursing experience preferred Hospice, home health, palliative care, oncology, geriatrics, or acute-care experience strongly preferred Strong clinical assessment and critical-thinking skills Excellent communication, organization, and time-management skills Ability to work independently in the field while collaborating effectively with an interdisciplinary team Compassionate and professional approach to patients and families Reliable transportation, valid driver's license, and automobile insurance Ability to travel throughout the assigned service area What We're Looking For The ideal candidate is an RN who is clinically strong, compassionate, organized, and dependable .
