Schedule:

13-week full-time contract Monday Friday, 8:00 AM 5:00 PM Expected caseload: 2532 points per week (generally 35 visits per day) Primary responsibilities: Conduct comprehensive OASIS assessments and develop individualized patient care plans. Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and management of chronic conditions. Monitor patient progress, promptly identify any health status changes, and communicate with the care team and physicians as needed. Educate patients and families on disease management, treatment, medications, and preventive health practices. Maintain timely, accurate documentation utilizing Home Care Home Base (HCHB). Collaborate with physical and occupational therapists, medical social workers, and home health aides for holistic patient care. Participate in case conferences, staff meetings, and ongoing educational opportunities. Ensure compliance with all regulatory documentation and care delivery standards. Travel daily to provide care within designated patient territory (mileage reimbursement included).

Qualifications:

Active RN license in good standing. At least 1 year of recent field-based home health experience. Proficient in use of HCHB and OASIS required. Strong clinical assessment and independent decision-making skills. Reliable transportation, valid drivers license, and auto insurance. Excellent time management, organization, and communication abilities. BSN preferred; understanding of Medicare and home health compliance is a plus.
