What You'll Be Doing Manage the

Care Journey:

Serve as the primary point of contact for a caseload of home health patients, ensuring seamless coordination of services.

Conduct Comprehensive Assessments:

Complete OASIS assessments and develop individualized care plans in collaboration with the interdisciplinary team.

Coordinate Services:

Work with physicians, nurses, therapists, social workers, and home health aides to ensure timely delivery of care and optimal outcomes.

Monitor and Evaluate:

Track patient progress, update care plans, and communicate with families and caregivers as needed.

Documentation:

Maintain accurate and timely clinical documentation in accordance with Medicare guidelines and agency policy. Advocate for the

What We're Looking For License:

RN (Registered Nurse) with active MA state license.

Experience:

At least 1 year of experience in home health or case management preferred. Expertise in administering IVs and medications.

Skills:

Strong critical thinking, communication, time management, and the ability to work independently.

Tech-Savvy:

Comfortable using EHR systems (experience with Kantime, Axxess, HCHB, or WellSky is a plus).

Reliable Transportation:

Valid driver's license, auto insurance, and access to a dependable vehicle.
