LCG is partnering with CareSource/Commonwealth Care Alliance on a clinical assessment initiative serving members across Massachusetts. We are seeking experienced, motivated RNs to join our MDS Assessment Unit. About the Role As an MDS Clinical Assessment RN , you will conduct comprehensive, face-to-face assessments in members' homes and community settings. Working within an assigned geographic territory, you will use the Uniform Core Assessment (UCA) , formerly the Minimum Data Set (MDS), along with supplemental assessment tools to evaluate members' clinical, functional, behavioral, and social needs. This is an excellent opportunity for an RN who enjoys independent clinical work, meeting members in their communities, and using clinical judgment outside of a traditional hospital or facility setting. You will spend part of your day in the field with members and complete your clinical documentation remotely. Your Schedule Assessments are scheduled Monday-Friday during the following appointment windows: 9:00 AM | 11:00 AM | 1:00 PM | 3:00 PM RNs must maintain 12 open appointment slots per week , with flexibility to arrange those slots across the workweek. A 4-day schedule may be available depending on territory and scheduling needs. Key Responsibilities Conduct Comprehensive Assessments Perform in-person MDS/UCA assessments in members' homes and community locations Complete supplemental tools including PHQ-9, EASI, AUDIT, and DAST Evaluate clinical, functional, behavioral health, and social needs Identify unmet needs and potential risks Document Accurately & Efficiently Complete SOAP notes, UCA/MDS documentation, Functional Assessments, and care interactions in GuidingCare Complete documentation within 48 hours Correct flagged assessments within 24 hours Maintain accurate, timely, and thorough clinical documentation Support Person-Centered Care Escalate urgent member needs appropriately Communicate findings to the care team Maintain confidentiality and comply with organizational policies Collaborate with multidisciplinary team members to support individualized care plans Use Technology Navigate electronic medical records and care management platforms Manage assessment schedules and appointment calendars Track assessment completion and communicate with team members electronically Utilize Microsoft Office, Outlook, Teams, and related technology Required Qualifications Associate Degree in Nursing (ADN) Active Massachusetts RN license in good standing Minimum 1 year of clinical experience Strong clinical assessment and decision-making skills Comfortable working independently Proficiency with electronic medical records Strong computer and organizational skills Valid driver's license Reliable vehicle and ability to travel throughout assigned territory Reliable internet connection for remote documentation Proficiency in English Experience with Microsoft Office, including Excel, Outlook, and Teams Preferred Qualifications Bachelor of Science in Nursing (BSN) Previous MDS-HC experience Experience with GuidingCare Home health, care management, or community-based assessment experience Bilingual skills What Makes You a Great Fit?
