Eliot Community Human Services seeks a compassionate Nurse Case Manager to coordinate care for adults and youth in community-based behavioral health and social service programs. This role conducts nursing assessments, develops person-centered care plans, and monitors medical and psychiatric needs across settings. The Nurse Case Manager collaborates closely with clinicians, social workers, and external providers, ensuring safe transitions of care, medication education, and health promotion. You will advocate for clients, engage families, maintain accurate documentation, and support crisis planning while contributing to an inclusive, mission-driven team culture focused on dignity and recovery. Responsibilities Perform comprehensive nursing and psychosocial assessments for clients in community-based programs Develop, implement, and update individualized, person-centered care and service plans Coordinate medical, psychiatric, and social services across providers and care settings Provide health education, medication teaching, and wellness support to clients and families Monitor client progress, safety, and symptom changes, intervening and escalating as needed Support discharge and transition planning from hospitals and higher levels of care Maintain timely, accurate clinical documentation and comply with regulatory standards Collaborate with interdisciplinary team members in care planning and case conferences Advocate for client needs, rights, and access to community resources Participate in crisis planning, risk assessment, and safety interventions as appropriate Required Skills Nursing assessment and clinical judgment Care coordination and discharge planning Behavioral health and psychiatric nursing knowledge Medication management and client education Interdisciplinary teamwork and collaboration Electronic health record (EHR) documentation Risk assessment and crisis intervention Motivational interviewing and engagement Knowledge of community resources and benefits Regulatory and compliance documentation skills
