HOSPICE RN CASE MANAGER - FALL RIVER AREA
Hanover Hospice is seeking a caring, experienced Hospice Registered Nurse (RN) to join our team in the Fall River area to provide coverage for a temporary leave. Flexible Scheduling ! We are looking for a compassionate, energetic, and detail-oriented RN who is dedicated to providing exceptional care and support to patients and their families during one of life's most important journeys. As a Hospice RN Case Manager, you will coordinate and provide individualized hospice care while working closely with patients, caregivers, families, physicians, and our interdisciplinary team.Responsibilities include:
Assessing patients and developing individualized plans of care Providing skilled nursing interventions and evaluating patient outcomes Managing and coordinating the overall plan of care for assigned patients Educating and supporting patients, caregivers, and families Collaborating with the interdisciplinary hospice team Maintaining accurate clinical documentation and meeting professional nursing standards Ensuring patients receive compassionate, respectful, and high-quality careWE ARE COMMITTED TO PROVIDING THE BEST CARE POSSIBLE!
