Job Description
The School Nurse Leader utilizes a coordinated school health approach to ensure that students are healthy, safe, and ready to learn through data collection and interpretation of health screenings, immunization, and disease surveillance metrics, and multidisciplinary student care coordination, communication, and planning with families, health care providers, communities, and staff.
Collaborates with students, families, and staff to write Individual Health, Medication, and Emergency Care Plans and contributes written health assessments to other plans (504, IEP) as needed. Assesses, provides basic treatment, and referral as requested for staff visitors to include preparation of the Supervisors Report of Accident-Intake Form.
Acts as Medication Program Manager and supervisor of medication storage, handling, delegation, and training for the District to include but not be limited to completing District Physician Standing Orders, MDPH applications for Controlled Substance Registration, and Federal Clinical Laboratory Improvement Amendments on an annual basis.
Serves as a health resource to families, students, and staff. Plans and implements staff and student health education programs in consultation with School Administrators, Counselors, and Staff.
Consults with Health and Wellness Advisory Committee and appropriate community leaders to review and update policies and procedures for district-wide comprehensive, coordinated health services.
Acts as liaison with District Physician, Hatfield Board of Health, HEADS Up, Hatfield Opioid Task Force, and regional Department of Public Health School Health organizations.
Supervises Nursing and Health Care Staff.
Manages district health-related grant programs as appropriate.
Completes records and reports for local, regional, and statewide needs as required.
These supports include the following:
adherence to current regulatory and evidence-based practices, the use of an information management system, and the provision of a range of prevention, risk assessment, and treatment services.RESPONSIBILITIES
Documents pertinent health records and reports for all students utilizing the SNAP electronic health record system.
